Overview

Licensed Practical Nurse (LPN) travel assignment in a Skilled Nursing Facility unit, located in Utica, New York. Start date: 11/02/2026. Duration: 13 weeks. Weekly pay range: $1,463 to $1,523 per week. 37.5 hours guaranteed per week. Shift: 2:00 pm-10:00 pm. This LTC position requires recent long-term care experience and works on a dedicated LTC unit with 40 beds. The assignment is travel- temporary and local-rate eligibility applies for nearby applicants.

Responsibilities

• Provide skilled nursing care to residents on the LTC unit under the supervision of an RN, following the plan of care
• Administer medications and treatments per physician orders; document care in the EMR (PointClickCare)
• Monitor residents' condition, identify changes, and communicate with the nursing team and interdisciplinary colleagues
• Assist residents with activities of daily living and support memory care and dementia residents
• Participate in discharge planning, admissions within the unit, and accurate record-keeping
• Adhere to safety, infection control, and privacy standards; participate in quality improvement and care coordination

Requirements

• Active LPN license in New York; Nursys verification required; NY license must be current
• Current BLS certification
• 1+ year of LTC facility experience
• EMR experience with PointClickCare
• Ability to work on a 2:00 pm-10:00 pm shift and float within the same building
• Driver's license with address that matches demographics
• RTO (time off) must be included at time of submission
• Local rate eligibility for applicants located within 75 miles
